Michigan Street Improvements <br />120-062 <br />CITY OF SOUTH BEND, INDIANA <br />Date: February 19, 2021 <br />To: All holders of the contract document for the above project. <br />The items in this addendum are to become a part of the original documents as if included therein. Only <br />the noted items are affected, and the remainder of the original contract document is to remain in its <br />entirety. <br />This Addendum modifies the Contract Documents as follows: <br />PART 1— GENERAL CLARIFICATIONS <br />PART 2 — SPECIAL PROVISIONS <br />PART 3 — PLANS <br />PART 1- CLARIFICATIONS <br />1.01 Attached are the meeting minutes and attendees sign -in sheet from the pre-bid meeting on <br />February 9, 2021, at 2 p.m. <br />1.02 Question: What is the thickness required of the concrete infill? <br />Clarification: Concrete infill shall now be HMA patching, Type B. The HMA patching, Pay Item <br />No. 20, shall be the same thickness as the existing HMA base and intermediate per detail 6, <br />sheet 11. It is listed as 10-1/2 depth, matching the COSB Standard Detail 2-11 Minimum <br />Pavement Section for Collector Streets. HMA surface shall be installed over HMA patching. <br />1.03 Question: Does fiber mesh need to be included in the concrete flatwork? <br />Clarification: No, fiber mesh and wire shall not be included concrete flatwork. Refer to note <br />2.07 in the section below for removal from Special Provisions. <br />1.04 Question: Is the concrete for the drive approaches 6" or 8" thick? Bid Tabs and details showed <br />differently. <br />Clarification: Per City standards, concrete drive approaches shall have a minimum 6" thickness <br />and shall not include wire mesh or rebar in the right-of-way. Refer to note 2.08 in the section <br />below for revision for Special Provisions. <br />1.05 Question: What are the landscape maintenance requirements? <br />Clarification: Contractor shall ensure seeded turf is established within a 90 day period and shall <br />be responsible for a minimum one (1) year guarantee time for the replacement of any plant <br />material which dies after the date of installation on site.
