<br />APPROVE PROJECT COMPLETION AFFIDAVIT -- NEW CONDUIT AND FIBER AT 319

<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Metronet Zing, 121 South Michigan Street, South Bend, Indiana 46601, for the
<br />above referenced project, indicating a final cost of $7,230.00. Upon a motion made by Ms. Roos,
<br />seconded by Mr. Inks and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER ISLAND
<br />PAVILION -- PROJECT NO ,111 -067 (SBCDA TIFF
<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit,
<br />Certificate of Completion and Fabric Roof Warranty on behalf Casteel Construction Company,
<br />Inc., 23186 Ireland Road, South Bend, Indiana 46614, for the above referenced project,
<br />indicating a final cost of $655,737.00. Upon a motion made by Mr. Inks, seconded by Ms. Roos
<br />and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT --- IGNITION PARK SOUTH
<br />DEMOLITION — PHASE III - PROJECT NO. 1,12 -040 (AEDA TIF)
<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Jackson Trucking and Excavating, 9067 West 100N, Kewana, Indiana 46939, for
<br />the above referenced project, indicating a final cost of $98,500.00. Upon a motion made by Mr.
<br />Inks, seconded by Ms. Roos and carried, the Project Completion Affidavit was approved.
<br />REPORT OF CONTRACT FINAL INSPECTION AND RECOMMENDATION FOR
<br />ACCEPTANCE — NORTH SIDE TRAIL EXTENSION — PROJECT NO. 110 -001
<br />(CMAQ /FHWA)
<br />Mr. Gilot advised that Thia Vawter, Engineering, has submitted the Report of Contract Final
<br />Inspection and Recommendation for Acceptance on behalf of Walsh & Kelly, Inc., 2.4358 SR 23,
<br />South Bend, Indiana 46614, for the above referenced project. Upon a motion made by Mr. Inks,
<br />seconded by Ms. Roos and carried, the Report of Contract Final Inspection and Recommendation
<br />for Acceptance was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — CENTURY CENTER WEST ENTRANCE IMPROVEMENTS-- PROJECT NO. 112-
<br />067 DOWNTOWN TIF
<br />In a memorandum to the Board, John Engstrom, Engineering, requested permission to advertise
<br />for the receipt of bids for the above referenced project. Also presented at this time for approval
<br />and execution was the Title Sheet. Upon a motion made by Mr. Inks, seconded by Ms. Roos and
<br />carried, the above request to advertise was approved, and the Title Sheet was approved and
<br />signed.
<br />ADOPT RESOLUTION NO 63 -2012 — DISPOSAL OF UNFIT AND /OR OBSOLETE
<br />PROPERTY.
<br />Upon a motion made by Mr. Inks, seconded by Ms. Roos and carried, the following Resolution
<br />was adopted by the Board of Public Works:

<br />A RESOLUTION OF THE SOUTH BEND BOARD OF PUBLIC WORKS
<br />ON DISPOSAL OF UNFIT AND /OR OBSOLETE PROPERTY
<br />WHEREAS, it has been determined by the Board of Public Works that the following property
<br />is unfit for the purpose for which it was intended and is no longer needed by the City of South
<br />Bend:
<br />WATER WORKS MISCELLANEOUS EQUIPMENT
<br />WHEREAS, Indiana Code 5 -22 -22 permits and establishes procedure for disposal of
<br />personal property which is unfit for the purpose for which it was intended and which is no longer
<br />needed by the City.
<br />NOW, THEREFORE, BE IT RESOLVED by the Board of Public Works of the City of
<br />South Bend that the items listed above are no longer needed by the City and/or are unfit for the
<br />
