• <br /> • <br /> (2) Said Clothing Allowance shall be paid by the City of South Bend in <br /> four (4) equal payments payable on or about March 15, June 15, September 15, and <br /> December 15, of each year. <br /> (3) All of the above described Clothing Allowance shall be pro-rated to <br /> the date of termination of employment for all qualifying Officers and Sworn <br /> Members. <br /> SECTION IV. INSURANCE BENEFITS. <br /> (a) Firefighters Contribution for Health Insurance. The contribution by <br /> Officer and Sworn Members by the South Bend Fire Department for single health <br /> insurance coverage for the self-funded insurance program of the City of South Bend <br /> shall be Ten Dollars ($10.00) per month in plan years 1991, 1992, and 1993. The <br /> contribution for dependent coverage shall be Fifty Dollars ($50.00) per month in <br /> plan years 1991, 1992, and 1993. <br /> (b) Comprehensive Major Medical Insurance. In an effort to control health <br /> care costs, the City's current health insurance package shall be modified beginning <br /> with the 1991 plan year to provide a program for Officers and Sworn Members of the <br /> South Bend Fire Department containing comprehensive major medical insurance <br /> which includes a Five Hundred Dollar ($500.00) annual deductible and which shall <br /> be subject to an 80/20 coinsurance payment to a maximum of Twelve Hundred <br /> Dollars ($1200.00) per year excluding any fee charged in excess of the then prevailing <br /> rates of medical services as established by the health insurance carrier for the City of <br /> South Bend. <br /> (c) Cafeteria Plan Contribution. The City of South Bend shall contribute Five <br /> Hundred Dollars ($500.00) to each Officer's and Sworn Member's individual health <br /> spending account. <br /> (d) Election to Leave the City's Insurance Program. The City of South Bend <br /> shall pay Thirty Dollars ($30.00) per month to any Officer or Sworn Member of the <br /> South Bend Fire Department who elects to leave the City's insurance program to be <br /> covered by another program for which the City makes no contribution. This <br /> election shall not be mandatory and the Officer or Sworn Member may return to the <br /> City's program provided that the conditions of the City's insurance plan are met and <br /> contributions specified in paragraph (a) herein of this Section are made, but such <br /> Officer or Sworn Member shall forfeit the Thirty Dollar ($30.00) payment per month. <br /> (e) Life Insurance. The City of South Bend shall provide term life insurance <br /> coverage in the amount of Ten Thousand Dollars ($10,000) to each member of the <br /> South Bend Fire Department. <br /> SECTION V. VACATIONS. The following Vacation Schedules, based on years of <br /> service, are recommended to the Board of Public Safety for approval: <br /> (a) 8-Hour Fire Personnel. All eight (8) hour Fire Personnel shall have <br /> comparable vacation time as 8 hour personnel in the Police Department with the <br /> same date of hire. <br /> (b) 24-Hour Fire Personnel. Officers and Sworn Members working twenty- <br />
