• <br /> member. Such forms shall include but not be limited <br /> to verification of the following information: <br /> 1. Complete legal name, address, and <br /> telephone number of the applicant. <br /> 2. Years of residency within the City <br /> of South Bend. <br /> 3. Political party if applicable to <br /> the appointment. <br /> 4. Qualifications as to why the <br /> applicant believes he or she would <br /> be a qualified candidate for the <br /> citizen position. <br /> 5. Summary of educational background, <br /> listing degrees and area of <br /> interest from any <br /> institutions of higher learning. <br /> 6. Summary of Employment History. <br /> 7. Involvement or participation in <br /> professional public service or <br /> other types of organizations. <br /> 8. Availability to attend meetings in <br /> the AM and PM. <br /> (e) All candidates who file a completed application <br /> form shall be scheduled for an interview with the <br /> Council President and/or Chairperson of the Council <br /> Committee or Council member appointed to serve on the <br /> board or commission related to the appointment in <br /> question. All other interested Council Members may <br /> attend such interview. <br /> (f) Persons selected by the Council as citizen <br /> representatives must have been residents of the City <br /> of South Bend for not less than one (1) year prior to <br /> the appointment and shall continue such residency as <br /> a qualification for said appointment. <br /> (ct) All such appointments shall serve without <br /> compensation for the term as indicated. <br /> (h) All citizen appointments shall agree as a <br /> qualification for appointment that he or she shall <br /> verbally report to the appropriate Council Committee <br /> and/or to the Common Council under Special Business <br /> at least once per calendar year with an overview of <br /> the items handled through the committee, commission, <br /> or other agency on which he or she participates. In <br /> the event of multiple Council citizen appointments to <br /> a committee board commission, or other agency, the <br /> Council citizen appointments shall rotate such <br /> reports among such persons. <br /> Ji) All application forms of persons not selected <br /> for a specific Council appointment shall be kept on <br /> file in the City Clerk's Office for a period of one <br /> (1) year in the event of a vacancy. <br /> Section II: This ordinance shall be in full force and <br /> effect from and after its passage by the Common Council and <br /> approval by the Mayor. <br />
