You must be tested no later than midnight (day) of <br />(Month), 20 .Our computer program selects the names of BCRC cardholders for <br />a random each month. You can be selected up to twice in a twelve month period. <br />In order to stay in compliance with BCRC and possess a clear (CL) BCRC card <br />status you will be required to report to an approved collection site within saen (7) <br />days from the date on this 'Notice of Random Testing'letter (the date on this letter <br />is included in the seven days). Please verify that the BCRC card number in the <br />upper right hand comer of this notice matches your BCRC card number. Ifit does <br />not match your BCRC card number, please notify our office immediately! <br />This random test can be taken on your oven time/taken on your <br />Emplo}^u/Contractor's time at any of the BCRC approv eoIlection sites <br />identified on the enclosed collection site list Failure to take your test by the date <br />outlined above will result in your BCRC earl status being deemcd'Not Available" <br />(N/A) and will require thatyou contact the BCRC office for instructions on the <br />required procedure(s) for conrcdnz the 'N/A status. In the event you are taking <br />prescribed mcdication(s), DO NOT take them to the collection site. You will be <br />contacted by our Medical Review Office (h1RO),who will needyour prescription <br />information for the verification process. please provide a legible and arcuate <br />telephone number on all forms at the collection site. <br />Remember, it is your responsibility to keepp yyour contact information current <br />with BCRC.
