Attachment B — Non -Bargaining: <br />BCRC LETTER HEAD <br />Letter of Notice for RandomTestig <br />Dear <br />You have been scicctcd by BCRCs computer generated random selection to take <br />a random drug test for BCRC. You must be tested no later than midnight (day) of <br />(Month), 20 .Our computer program selects the names of BCRC cardholders for <br />a random each month You can be selected up to price in a twelve month period. <br />In order to stay in compliance with BCRC and possess a clear (CL) BCRC card <br />status you rill be requited to report to an approved collection site within seven (7) <br />days from the date on this "Notice of Random Tesfing'lettcr (the date on this letter <br />is included in the satn days). Please verify that the BCRC card number in the <br />upper right hand comer of this notice matches your BCRC card number. If it does <br />not match your BCRC cud number, please notify our office immediately! <br />This random test is to be taken on your oven time at any of the BCRC approved <br />collection sites identified on the enclosed collection site list Pailure to take your test <br />the date outlined above will result in your BCRC card status being deemed <br />ot Available" (N/A) and will require that you contact the BCRC office for <br />instructions on the required proccc durc(s) for correcting the "N Xstatus.In the <br />event you are taking prescribed mcdicadon(s), DO NOT take them to the <br />collection site You will be contacted by our Medical Review Office (MRO), who <br />will need your prescription information for the verification process. Please provide <br />a legible and accurate telephone number on all forms at the collection site. <br />Remember, it is your responsibility to keepp your contact information current with <br />BCRC.
