<br />APPROVE CHANGE ORDER NO. 1 — KIZER MANSION WINDOW REPAIR — PROJECT
<br />NO. 119-024R (PO-0000204)
<br />Mr. Gilot advised that Mr. Zach Hurst, Engineering, has submitted Change Order No. 1 on
<br />behalf of J.W. Werntz & Son, Inc., 1002 Kerr St., South Bend, IN 46601, indicating the contract
<br />amount be decreased by $21,000 for a new contract sum, including this Change Order, in the
<br />amount of $197,000. Upon a motion made by Ms. Maradik, seconded by Mr. Molnar and carried
<br />by roll call, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO.4 — PULASKI PARK IMPROVEMENTS — PROJECT NO.
<br />118-056 (PO-0001782)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 4 on
<br />behalf of Larson -Danielson Construction Co, Inc., 302 Tyler St., LaPorte, IN 46350 indicating
<br />the contract amount be increased by $2,001, and an additional one -hundred, ninety (190) days,
<br />for a new contract sum, including this Change Order, in the amount of $898,527.75 and a new
<br />completion date of June 27, 2020. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Molnar and carried by roll call, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — PINHOOK PARK SITE IMPROVEMENTS —
<br />PROJECT NO. 118-077A (PO-0001779)
<br />Mr. Gilot advised that Ms. Alicia Czarnecki, Engineering, has submitted Change Order No. 2 on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614,
<br />indicating the contract amount be decreased by $24,556.76 for a new contract sum, including this
<br />Change Order, in the amount of $1,263,868.24. Upon a motion made by Ms. Maradik, seconded
<br />by Mr. Molnar and carried by roll call, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— PRIVET LANE CULVERT REPLACEMENT PROJECT — PROJECT NO. 118-013B (PO-
<br />0000302)
<br />Mr. Gilot advised that Ms. Rebecca Plantz, Engineering, has submitted Change Order No. 1
<br />(Final) on behalf of HRP Construction, Inc., 5777 Cleveland Rd., PO Box 266, South Bend, IN
<br />46624, indicating the contract amount be decreased by $3,883.22, with an additional ninety (90)
<br />days, for a new contract sum, including this Change Order, of $260,587.78. Also submitted was
<br />the Project Completion Affidavit indicating this new final cost of $260,587.78 with a revised
<br />completion date of June 30, 2020. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Molnar and carried by roll call, Change Order No. 1 (Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 315 & 319 W. JEFFERSON EXTERIOR RENOVATIONS, DIVISION C — PROJECT NO.
<br />119-064 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, IN 46614, indicating the
<br />contract amount be decreased by $4,770 for a new contract sum, including this Change Order, of
<br />$43,075. Also submitted was the Project Completion Affidavit indicating this new final cost of
<br />$43,075. Upon a motion made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call,
<br />Change Order No. 2 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — SMALL DRAINAGE DESIGN AND
<br />REPAIR — SAMPSON & OVERLOOK — PROJECT NO. 118-063A (LRSA)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of HRP Construction, Inc., 5777 Cleveland Rd., PO Box 266, South Bend,
<br />IN 46624, for the above referenced project, indicating a final cost of $346,638.91. Upon a
<br />motion made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call, the Project
<br />Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — EWING AVE. AND FELLOWS ST.
<br />TRAFFIC SIGNAL REPLACEMENT — PROJECT NO. 118-097 (LRSA)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Martell Electric, LLC, 4601 Cleveland Rd., South Bend, IN 46628, for the
<br />above referenced project, indicating a final cost of $188,858.54. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Molnar and carried by roll call, the Project Completion Affidavit was
<br />approved.
