25% and alcohol tested at 10% every twelve months, with testing again to be conducted on a <br />quarterly basis. <br />The current DOT Federal Highway Administration requirements for people with CDL licenses <br />are required to be tested at the rate of 25% annually. In addition, the company intends to apply <br />this same ratio to all other employees of the company. Tests for both groups will be <br />unannounced and based on a computerized random selection from both pools. <br />Post Incident / Injury Testing: <br />Any covered employee working within safety sensitive guidelines and whose performance either <br />contributed to an incident or an injury and cannot be discounted as a contributing factor to an <br />incident, will be drug and/or alcohol tested under the following guidelines if the incident meets <br />any of the following criteria: <br />DOT Pipeline Jobs: <br /> An event that involves a release of gas from a liquid pipeline or of liquefied natural gas <br />or gas from an LNG facilities; <br /> Death or personal injury necessitating in-patient hospitalization or estimated property <br />damage, including cost of gas loss to the operator of others or both of $50,000 or more; <br /> An event that results in emergency shut-down of an LNG facility; <br /> An event that is significant in the judgement of the operator. <br />DOT Federal Highway Administration: <br /> Post incident testing is required for employees with CDL’s that were involved in <br />reportable driving related incidents such as: <br /> When any fatality occurs as a result of an incident, or <br /> When a company driver is issued a citation under state or local law for a moving <br />violation arising from an incident. <br />Company Additional Requirements: <br /> One or more motor vehicles incurring physical damage due to the operation of a company <br />owned vehicle operated by an active employee. <br /> Any job-related injury requiring medical treatment. <br /> An event of significance in the judgement of the company even if it does not meet the <br />above criteria. <br /> An event that is significant in the judgment of the employer (i.e., substantial damage to <br />company, customer or public property). <br /> Employee(s) must be escorted by a supervisor to the collection facilities. The breath <br />alcohol testing ideally must be administered within two (2) hours after the incident, but <br />no longer than eight (8) hours after the incident.
