Wall A is always the wall that is closest to <br />the address elevation or the "street side" of the house. Moving <br />clockwise, the walls are then B, C, D. <br />Windows are provided a numerical number, which is reflected <br />on the site plans included in the Lead Inspection & Risk <br />Assessment Report. <br />LBPGC10 BUILDING PERMIT REQUIRED <br />The contractor is responsible for submitting this owner-prepared <br />work write up to the building department, applying for, paying for <br />and receiving a building permit(s) prior to starting any work. <br />LBPGC11 OWNER RESPONIBILITIES <br />The owner shall provide: <br />1. Utilities and sanitary facilities. <br />2. Remove all personnel items from work area. <br />3. All full and complete access to work areas. <br />4. Allow site access during normal work areas. <br />LBPGC12 STABILIZE NON-LBP PAINT <br />Deteriorated non-LBP has been identified in association with the <br />structure, to ensure proper cleaning and prevent lead-dust <br />hazards from non-LBP that contains lead below HUD/EPA <br />regulatory limits, loose paint shall be adequately removed prior <br />to final cleaning. After establishing any required floor <br />containment with polyethylene sheeting, mist defective paint <br />area with water. Aggressively remove all loose paint or <br />deteriorate substrate with a stiff bristled brush or broom. <br />Contractor may elect to: Repair any damaged substrate and <br />spot prime areas of deteriorated non-LBP paint to ensure that <br />paint does not become loose prior to clearance. <br />Project will fail clearance if any paint chips, dust or debris are <br />identified, regardless of source. <br />1.00 EA job $950 <br />1.00 DU n/a n/a <br />1.00 DU Job $300 <br />Location Total:$7,150.00 <br />Page 3 of 24
