INC. <br />1703 South Ironwood o South Bend, IN 46613 <br />November 14, 2017 <br />Phone: (574) 235-5931 <br />Attn: Patrick Kerr <br />Email: rwilkers@southbendin.eov <br />Project Location: 115 Lafayette Blvd <br />We wish to submit a bid on the project above using the following specifications: Complete tear off <br />and re -roof using the Johns Manville (JM).060m1 TPO Rhino Bond system (See attached brochure) <br />Scope of Work <br />Preparation <br />• Upon execution of contract, arrange a preconstruction meeting with the client and/or their <br />representatives to discuss start date, special conditions, staging of material and waste <br />containers, working hours and all other necessary information to proceed. <br />• Secure all necessary building permits to proceed with project. <br />• • Project is bid. as tax exempt. <br />• Using a local licensed HVAC contractor, disconnect the existing roof top units. <br />• Using our .crane, remove the units to the ground level to facilitate the deck replacement and <br />installation of the new roofing system. <br />• Upon completion of the roofing system, re -set the units and re -connect. <br />• At the east wall where the spalled brick is located, mechanically attach new CDX plywood to <br />provide a solid substrate for the new flashings. <br />—--• Remove all existing crock tile coping caps. Install new wood blocking to receive the edge metal <br />detail. <br />Demolition <br />• Remove the existing membranes, edge metals and all roof top flashings. All roofing debris will <br />be disposed of following local rules and regulations at a properly licensed landfill. <br />• Check for are replace if found, any bad decking with like kind and thickness at an additional cost <br />of $2.75 per square foot for OSB/CDX decking (minimum. of 16 sq. ft.) and $3.40 per In. ft for 1" <br />x deck boards and/or fascia (minimum of 8 In. ft.). Tongue and groove decking, roof joists <br />and/or trusses will be billed on a time and material basis of $80.00 per man hour and material <br />costs plus 20% markup for overhead and profit. <br />Insulation Assembiv <br />• At all drain locations, mechanically attach the JM Tapered Panels using the JM Rhino Plates and <br />approved fasteners extending 4 feet in each direction to create a sump for better drainage. <br />• Over the remainder of the exposed decking, mechanically attach 1 layer of JM 2" <br />Polyisocyanurate insulation using the JM Rhino Plates and approved fasteners.
