4. The inst"n of twelve new street lights (see plmmiiiDr type of pole, fixture, and <br />placemenwi r ations) along the curbed edge of the --parking areas. The conduit <br />containing the electric service shall be three (3) inch PVC Conduit at a minimum <br />depth of three (3) feet. The Contractor will be responsible for the exact placement <br />of the conduit as to not effect any trees that remain in the parking area. The electric <br />feed will be obtained from the existing line located on a pole near the bridge to <br />Leeper Island (as shown on the plans). The City will contact AEP prior to <br />construction for placement of a meter. <br />5. Compact fills and subbase materials 100 % compaction. Testing will be done by <br />nuclear gauge. Testing and inspection will be accomplished by the Department of <br />Public Works. The department personnel are certified for earthwork and bituminous <br />inspection and testing by INDOT. <br />XXI. TEMPORARY EROSION CONTROL, <br />A. Prevailing Specifications: 1999, INDOT Standard Specifications Section 205 <br />B. Additions: <br />1. The Contractor shall be responsible for the preparation of the Erosion Control Plan, <br />as required by 107.14.1 of the Prevailing Specifications. Temporary Erosion Control <br />shall include the cost of the preparation of the plan, payment of fees and charges <br />associated with the filing ofsaid permit and legal advertisement, and implementation <br />of the plan, including maintenance of the erosion control elements. Should the <br />Contractor fail to' comply with the requirements of any agency having lawful <br />jurisdiction over the effectiveness of the Erosion Control Plan, or fail to adequately <br />implement or maintain the the elements of the plan, the Contractor shall be solely <br />responsible for fines, imprisonment or any other penalties lawfully imposed <br />therefore. <br />XXII. HOT MIX ASPHALT (HMAI: <br />A. Prevailing Specifications: 1999, INDOT, Standard Specifications Section 402. <br />B. Additions to Section 402: <br />1. The CONTRACTOR shall submit a job mix formula for approval five (5) calendar <br />days prior to any mix placed. The contractor shall notify the City 48 hours prior to <br />placing any asphalt. Mix placed without notification will be subject to no payment. <br />2. The CONTRACTOR shall hire an outside approved testing laboratory to perform <br />testing. Test results shall be given to the City within 48 hours. A City representative <br />shall be present and designate a random location for sample. A representative from <br />testing lab shall take immediate possession of sample taken and sign City form. <br />3. A minium of one (1) plate sample, for each type of mix placed shall be taken for each <br />day that 50 tons is placed, for more than 500 tons per day, one (1) sample for each <br />500 tons. <br />
