Version 6-8-2017 <br />APPENDIX "B" <br />INFORMATION AND SERVICES TO BE FURNISHED THE LPA: <br />The LPA shall furnish the CONSULTANT with the following: <br />1. Standard Specifications and standard drawings applicable to the project <br />2. Plans of existing bridge within the project limits <br />3. Necessary permit forms and permit processing (US Army Corps of Engineers, US Coast Guard, <br />and/or Indiana Department of Natural Resources) <br />4. Utility plans available to INDOT covering utility facilities govern the location of signals and <br />underground conduits throughout the affected areas <br />5. Provide access to enter upon public and private lands as required for the CONSULTANT to <br />perform work under this Contract <br />6. Geotechnical investigation, if applicable <br />22 <br />
