Change Order No: l2 to Contract No 5, Phase III: CHANGE ORDER NO. 12 <br />Thi Change Order is for an increase in contract amount of TO CONTRACT NO. 5, <br />$2,292.92,' with The Hickey Company, Inc., R -66. PHASE III APPROVED, <br />Mr. Bill Slabaugh gave a breakdown of the work involved: R -66 <br />ault at Sherland Building had glass <br />locks in top which were previously <br />idden. The glass blocks could not <br />accept resurfacing and had to be re- <br />moved. <br />$ 1,382.30 <br />Vault curbs capped - -182 linear feet of <br />deteriorated curbs. <br />1,090.20 <br />ault originally scheduled for demoli- <br />ion prior to this contract. Not carried <br />ut as yet, to be performed in this con- <br />tract. Extra work to reinforce the in- <br />ide wall of vault prior to backfilling. <br />1,131.60 <br />National Bank requested they be allowed <br />to resurface their own vault due to <br />eating elements in concrete. <br />Credit.... <br />( 606.00) <br />I M vault at St. Joseph Bank in good <br />c ndition.
