General The City of South Bend, Indiana seeks to implement Cityworks Permitting, Land, Licensing (PILL) as their system of record to managing their business licensing processes and data. This Statement of Work (SOW) has been developed in response to South Bend's requirements, existing system documentation and data. Woolpert's Permitting System implementations are focused on delivering a comprehensive set of technology tools and best practices that support our clients' performance objectives. We achieve our clients' objectives by first understanding their Implementation Vision and then defining the Management strategies to achieve the vision. We then design and deploy the System Applications required to support sustained Management & Monitoring. There are many related Tasks and Sub -Tasks that must be properly executed to ensure a successful deployment of high -value Permitting Systems. Woolpert's project methodology fully addresses all aspects of this complex project and ensures all critical success factors are thoroughly addressed in a logical order. Figure 1- PILL Implementation - Guiding Principles Services, and associated deliverables, are categorized into five (5) major phases, as more fully detailed in the following sections. Orderly completion of the Tasks and Sub -Tasks within each phase culminates with the execution of a Phase Acceptance and Close document signifying the Client's acceptance of the services rendered to date and authorization for Woolpert to proceed with work on the subsequent Phase. Our project plan and associated budget and schedule were developed using the following overarching assumptions, which were taken from communications with members of your organization. Task -specific assumptions used to develop the detailed services and deliverables related to each task in our project plan are noted throughout this document against the appropriate task. • Cityworks PILL will be implemented for the South Bend Department of Community Investment — Business Licensing Division • The Client is to identify and empower a single Project Manager to manage the PILL implementation team. • The Client is to identify and empower power users, a Cityworks System Administrator, and Crystal Report Writer (from Departments of Community Investment and Innovation & Technology) who, along with the client Project Manager, will collectively constitute the Client Core Team. The Client Core Team will be formally documented in the Project Plan. • Woolpert will configure Cityworks PILL to manage twenty-seven (31) business license types as listed below: Adult Business License Arborist License Charitable Solicitation License Food Vending Machine License Hotels and Motels License Lawn Parking Massage Therapist License Open -Air License Peddlers and Canvassers License Precious Metal Dealer License Alarm Agent License Automotive Repair License Donation Box and Container License Food Vending Vehicle License Itinerant Restaurant License Massage Establishment License Mobile Food Vendor License Outdoor Motion Picture Theater License Pool Hall License Public Parking Facility License SOUTH BEND, IN - BUSINESS LICENSING, PLL IMPLEMENTATION SERVICES I SCOPE OF WORK
