Please review the project scope, location(s), and Grantee address listed carefully, and if you have no questions, concerns, or corrections with the agreement, please: • Return one 11 original executed agreement to my attention for processing within sixrt(1) Weeks of receipt. o Local Public Agency (LPA) must sign and return the LPA-INDOT Grant Agreement no later than two (2) months from the date of this award letter. o Signatures cannot be over 30 days old once it reaches the INDOT LPA/MPO Division Office located in Indianapolis. o Failure to meet this deadline will result in forfeiture of your funds. • Review the project scope, location(s), and the Grantee address in the contract. • Print the agreement: portrait, single sided, and do not staple. o Be sure the page umbering cgg1ches the pff file when printed. • Print or type the name and title of the LPA individual signing the agreement. o Statutory Authority (Must _ e s j&r gcl 1)r Path a : Mayor, Town Board President or Manager, City Council President, or Commissioner). o Signature must be dated. • p stamp1 ill be aecgptc l alit) walll t q e N l e nl wwlaerL �uw „. o This is a legal document, and no -chances shall be made to the_agreement by the LPA.
