APPENDIX E <br />NOTICE TO PROSPECTIVE CO-WORKERS <br />A Post -Offer Physical is a requirement for some positions with High companies. One of the <br />requirements for consideration of employment is the satisfactory passing of a Drug Screen Test. <br />Please read the following information and instructions carefully: <br />NOTICE: This Company has a policy which prohibits the possession and/or use of illegal <br />and unauthorized drugs and alcohol. The Company will search and substance screen its <br />co-workers' personal effects as an enforcement measure of this policy in order to maintain <br />a safe, healthy, productive working environment. Certain business units within the <br />corporation have also elected to conduct random substance abuse testing. This policy is <br />enforced to maintain the integrity and security of Company facilities and property. You <br />may be eliminated from consideration from employment with a High company if the <br />results of your substance screen indicates that you are applying for employment while you <br />are using alcohol or any unauthorized or illegal drugs. <br />NOTE TO APPLICANT: The laboratories which conduct the screenings will automatically <br />run two screenings for any positive result, using two separate methodologies. The first is a <br />screening test, the second is a confirmation test. <br />For a positive drug test result, if you desire, a portion of the specimen will be made available to a <br />laboratory of your choice for additional confirmation testing. If you request this to be done, you <br />must bear the expense of the additional testing if the results of the original testing are confirmed. <br />APPLICANT: I have read and understand this phase of the post -offer medical requirements. I <br />accept the conditions for consideration of employment. I consent to the requirements of the <br />substance screens and any other post -offer physical examination if required. I agree in <br />submitting to these tests that the testing facility is authorized by me to provide the test results to <br />High. I consent freely and voluntarily to the Company's request for necessary specimens.
