APPENDIX D <br />/i <br />CONFIDENTIAL <br />APPLICANT/CO-WORKER CONSENT FORM - ILLEGAL DRUGS, ALCOHOL, <br />CONTRABAND AND UNAUTHORIZED ITEMS <br />In order to assist in maintaining a safe working environment for co-workers, as an enforcement <br />measure in providing a safe, healthy, productive work environment and to maintain the integrity <br />and security of Company facilities and property, High, has established a Policy No. 2-662 <br />(hereinafter referred to as "the Policy") regarding drugs/alcohol use and/or abuse, on any High, <br />affiliated or subsidiary, premises, property, facilities, installations, work locations, work areas, <br />vehicles, equipment, boats or aircrafts (whether owned, leased or used). <br />I understand that the Company may implement the Policy by such reasonable means as the <br />Company may from time to time determine appropriate, including searches and inspection of <br />personal effects and drug/alcohol testing, of any person according to the guidelines in the Policy. <br />Co-workers who either decline to be searched, decline to submit to drug/alcohol tests, as required <br />in the Policy, or who otherwise are found in violation of the Policy, will be subject to suspension <br />with intent to discharge from employment. <br />Prospective co-workers who refuse to submit to drug/alcohol testing as part of or separate from a <br />post offer examination, or who are otherwise found to be in violation of Policy No. 2-662, will <br />be disqualified for further employment consideration. <br />I understand a documented chain of specimen custody exists to ensure the identity and integrity <br />of my specimens throughout this collection and testing process. <br />ACKNOWLEDGMENT <br />I have read and understand the Policy, and agree to abide by all of its requirements, including <br />drug/alcohol testing.
