All persons who are <br />eligible to obtain Northern Indiana NECA/LU 153 drug -free status will be instructed to report to <br />a pre -selected urine collection site within a pre -determined time. These selected sites will have <br />on hand collection kits, chain -of -custody forms, and all other necessary supplies. Whenever an <br />individual is directed to a collection site, the individual will proceed to that site directly after <br />work. <br />Page 7 of 31 <br />
