c. Final report documenting the completion of the project with final air clearance <br />certification. <br />d. The Contractor is to perform daily air sampling to document that its wet and other site <br />cleanup work techniques are complying with all regulatory agency guidelines and <br />requirements for no visible emissions and dust concentrations being maintained below <br />the OSHA Permissible Exposure Limit (PEL). <br />e. Perform daily on -site airborne nuisance dust air monitoring services during normal <br />work activities. <br />5. In all cases where it is necessary to work in the public right of way, including the sidewalks <br />and alleys, the Contractor shall submit a traffic control plan to the Engineering Department <br />using the attached form for approval a minimum of five (5) working days prior to beginning <br />the work at the structure. The Contractor will provide all barricades, warning signs and <br />lights, flaggers, or other methods in compliance with the Indiana Manual on Uniform Traffic <br />Control Devices to control traffic. Alleys shall be used wherever possible for truck and <br />equipment access to the site. No tracked vehicles shall be operated on public streets or <br />public sidewalks. Loading of debris into trucks located within the right of way will not be <br />approved without proof that there is no other way to accomplish the work without damage <br />to private property. <br />6. Payment for debris removal shall be paid for at the contract unit price for "DEBRIS <br />REMOVAL...... LSUM° as indicated on the itemized proposal sheet. <br />•:W-7,111MY <br />IMUMOMPTEM <br />A. Prevailing Specifications: 2018, INDOT Standard Specification Section 110 <br />B. Additions: <br />1. The mobilization portion of this item will be limited to 75% of the Lump Sum Price. The <br />balance of the Lump Sum Price will be considered demobilization and will be paid for when <br />all work, including final cleanup, is completed. <br />2. Mobilization and demobilization will be paid for at the contract unit price for <br />"MOBILIZATION AND DEMOBILIZATION......
