This license must be available for review at all times while <br />you are implementing an asbestos project. <br />This license may be revoked, pursuant to 326 IAC 18-1-7, if you: <br />(1) Violate any requirements of these rules (326 IAC 18), 326 IAC 14-10, or any requirement of the <br />Asbestos -Containing Materials in Schools Rule or any other federal, state, or local regulation pertaining <br />to asbestos in buildings or to asbestos projects. <br />(2) Falsify information on your application for licensing. <br />(3) Fail to meet any qualifications specified in 326-IAC 18-1-4. <br />(4) Conduct asbestos project, or related asbestos handling activity, in a manner which is hazardous to the <br />public health. <br />Your license is valid effective 06/08/2019, and will expire on 06/08/2020, as indicated on your card. We <br />suggest that you attend the required training and submit an application for license renewal within 60 days of <br />your expiration date to insure your license does not lapse.
