N Unicipat Utilities and Service Lines ........ Rules anggulLions <br />service charges for two months of service when: <br />(1) The customer has been mailed disconnect notices <br />for more than two (2) consecutive months; <br />(2) The customer has been mailed disconnect notices <br />for any three (3) months within the preceding <br />twelve (12) month period; or <br />(3) The service to the customer has been disconnected <br />pursuant Section VIII. - DISCONNECTION OF <br />SERVICE. <br />The amount of any such security deposit may not exceed <br />an amount equal to one -sixth (1/6) of the expected annual <br />billings of the customer at the address at which service is <br />rendered. In the event the required deposit is greater <br />than seventy dollars ($70.00), the Utility shall advise the <br />customer that they may pay such a deposit in equal <br />installment payments over a period of up to four (4) <br />weeks, except where such deposit is required as a result of <br />a disconnection for nonpayment, in which case the deposit <br />may be required prior to reconnection. <br />3. Record of De ostt�. <br />a) A record of each deposit shall be maintained showing the <br />following: <br />(1) The name of the customer. <br />(2) The current address of the customer, so long as the <br />customer maintains an active account with the <br />Utility in their name. <br />(3) The amount of the deposit. <br />(4) The date the deposit was made. <br />(5) A record made of each transaction affecting such <br />deposit. <br />b) Each customer shall be provided a written receipt from <br />the Utility at the time their deposit is paid in full or when <br />they make a partial payment. The Utility shall also <br />provide a reasonable method by which a customer can <br />establish that they are entitled to a refund. <br />4. Review of Account <br />Any customer currently being provided service may make a <br />written request to have the Utility review the customer's deposit <br />amount. If the customer is deemed creditworthy, then the <br />Utility will credit any deposit to the customer's account. <br />5.
