We shall have <br />no responsibility to inspect for, or remove, hazardous materials. <br />We will perform these services in a manner consistent with the degree of care and skill ordinarily <br />exercised by members of the same profession currently practicing under similar circumstances. <br />PROJECT DELIVERABLES <br />1. Well pumping summary <br />2. New delineation(s) <br />3. Updated PSC Inventory, including: <br />a. PSC maps on aerial photography images depicting wellhead protection area boundaries, <br />potential sources of contamination, and general land use. Hard copy and PDF maps will be <br />provided. If requested, ARCNiew shapefile data will be provided. <br />b. Tabular summary of potential sources of contamination identified within each wellfield <br />4. Updated Management Plan, including summary of potential source management actions taken <br />over the past five years <br />5. Updated Contingency Plan <br />6. Local Planning Team documentation <br />7. Public Outreach and Educational product <br />8. Completed IDEM Phase II Five -Year Update Survey form (Deliverables 1-5 will be supporting <br />documentation.) <br />The services and deliverables described above will be completed according to the schedule outlined <br />below. Assuming notice to proceed is received by June 1, 2019, the final report would be completed by <br />January 1, 2020. <br />COMPENSATION <br />Compensation for services rendered will be lump -sum and invoiced monthly on a percent -complete <br />basis. Full payment of invoices is due within 30 days from invoice date. The fee amounts are as follows. <br />Wellhead Protection 5-year Update.. .. ......... ............. ....... ................. ............... $24,850 <br />If payment is not made within 30 days of the date when the payment is due, we may, at our option, and <br />effective upon the delivery of written notice of our intention to do so, terminate the contract or suspend <br />further performance of our services under the contract, and we shall have no liability for delay or damage <br />that results from the termination of the contract or suspension of services. <br />
