Prepare a tabular summary of PSCs, including any new PSCs and notation of the closure or <br />delisting of PSCs that no longer present a threat. The table will include: <br />i. Facility identification number <br />ii. Facility name and location <br />iii. Site description <br />iv. Any environmental permits issued for the site, including permit number and issuing <br />agency <br />v. Types of contamination at the site <br />vi. Operating status of the site <br />4. Potential Source Management <br />Pursuant to the IDEM Wellhead Protection 5-year update reporting form, a summary of actions <br />the South Bend Water Works has taken to manage the sanitary setback area and the overall <br />wellhead protection area are to be summarized. As an initial step, we will evaluate the measures <br />proposed in the wellhead protection plan and identify measures that have been completed or are <br />currently being implemented. The general management plan components addressed in the Phase <br />I wellhead protection plan include: <br />a. Prevention of contamination within the sanitary setback or well -isolation area <br />b. Identification of specific management or monitoring measures that consider the location and <br />type of potential sources of contamination <br />c. Coordination of the system's monitoring plan with the types of potential sources of <br />contamination in the wellhead area <br />d. Identification of abandoned wells <br />e. Development of procedures for maintaining and updating the potential source inventory <br />f. Notification of property owners located within re -delineated wellhead protection area(s) <br />g. Materials for public outreach program <br />We will compile the information needed to document management and monitoring measures <br />proposed in the wellhead protection plan that have been completed or are currently being <br />implemented. Some examples include the ongoing site permitting and inspection program <br />implemented by the St. Joseph County Health Department. <br />In some cases, proposed management/monitoring measures may not be completed or are not <br />achievable. In these cases, we will work with South Bend staff to outline an updated <br />implementation plan or develop alternate measures that can be implemented within the next five <br />years. A summary of the implementation efforts, as well as copies of supporting documentation <br />and an updated implementation plan, will be prepared and incorporated into the 5-year update <br />summary. <br />
