<br />Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the above
<br />proposals were referred to Venues, Parks and Arts for review and recommendation.
<br />OPENING OF QUALIFICATIONS — QUALIFIED CONTRACTORS FOR LEAD HAZARD
<br />REDUCTION DEMONSTRATION GRANT (IHCDA STATE GRANT)
<br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed qualifications
<br />for the above referenced grant. Mr. Gilot stated the qualifications will be reviewed and scored
<br />based on a matrix system, by a team made up of City representatives. He read the names of the
<br />following companies submitting qualifications:

<br />Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the above
<br />qualifications were referred to Community Investment for review and recommendation.
<br />EXTEND AWARD BID — EIGHT (8) MORE OR LESS CARDIAC
<br />MONITORS/DEFIBRILLATORS (EQUIPMENT PURCHASES/MATERIALS &
<br />EQUIPMENT)
<br />Mr. Andy Myer, Fire Department, advised the Board that on May 22, 2018, bids were received
<br />and opened for the above referenced equipment. On June 12, 2018, Mr. Myer recommended that
<br />the Board award the contract to the lowest responsive and responsible bidder Physio-Control,
<br />Inc., 11811 Willows Road NE, Redmond, WA 98052 in the total amount of $208,147.92. On
<br />May 14, 2019, Mr. Myer requested an additional thirteen (13) cardiac monitors/defibrillators be
<br />awarded with extended unit price, for a total of $309,835. Therefore, Ms. Maradik made a
<br />motion that the recommendation be accepted and the bid be awarded as outlined above, Ms.
<br />O'Sullivan seconded the motion, which carried.
<br />AWARD BID — WATER TREATMENT CHEMICALS 2019-2020 (VARIOUS
<br />OPERATIONAL BUDGETS)
<br />Mr. Bradley VanOosterum, Utilities, advised the Board that on April 23, 2019, bids were
<br />received and opened for the above referenced chemicals. After reviewing those bids, Mr.
<br />VanOosterum recommended that the Board award the contract to the lowest responsive and
<br />responsible bidders in the amount of unit prices:
