2. "Reasonable Cause" Testing: <br />Employees may be required to submit to a controlled substance screening test any time <br />supervisors or management have a good reason to believe an employee is working with <br />drugs and/or alcohol in his or her system. Example of "reasonable cause" include, but <br />are not limited, to the following: <br />a. Slurred or incoherent speech; <br />b. Glassy -eyed stare; <br />C. Disoriented behavior; <br />d. Unstable gait; <br />e. Lack of motor control; <br />f. Noticeable poor work performance (e.g. frequent accidents, absenteeism, <br />abuse of sick leave, etc.); <br />g. Unexplained increase in absenteeism and/or tardiness; <br />h. Involvement in accidents on company property that result in personal <br />injury or property damage; and <br />i. Extreme change of behavior. <br />EMPLOYEES WHO TEST POSITIVE OR WHO REFUSE TO SUBMIT TO A <br />CONTROLLED SUBSTANCE SCREENING TEST MAY BE TERMINATED <br />IMMEDIATELY. <br />If an employee tests positive, we will allow the employee to have a part of the sample re -tested <br />at his or her own expense within ten (10) days after notification of the test results by Holladay. <br />Any employee who tested positive for drugs or alcohol in Holladay's drug test may provide <br />and explanation of the positive test and/or request a confirmatory test. Employee is responsible <br />for bearing the cost of the confirmatory re -test. <br />10 <br />
