<br /> Brandon Adolf, 2734 Sampson Street, South Bend, IN, served as a petitioner for this legislation.
<br /> Mr. Adolf stated, First (1St) and foremost, thank you, ladies and gentlemen, for letting us come
<br /> here tonight. I will go over some of the information in that packet and kind of do an introduction.
<br /> Choices Recovery, we are an in-patient drug and alcohol treatment facility. We are State-licensed
<br /> and CARF-Accredited, which is the highest accreditation level a rehab facility can have. We
<br /> recently did that survey the 10th and 11th of December 2018. We passed that survey with a three
<br /> (3) year accreditation, which is the highest level you can get through that. Choices currently
<br /> services people from a wide variety of life. We do everything from medical detox to intensive out-
<br /> patient. We offer evidence-based treatment based off of ASAM Criteria including one (1) on one
<br /> (1)case management and medical detox by our Medical Director of Addictionology,Emma Wong.
<br /> Mr. Adolf continued, Again, like I stated, Choices Recovery has been operating for a little over
<br /> four (4) years at 3606 East Jefferson Boulevard,just point three (0.3) miles down the street from
<br /> this proposed location. In 2018,Choices had five hundred and forty-one(541)successful graduates
<br /> from its program. It serviced two hundred and forty-one (241) clients from the State of Indiana
<br /> and one hundred and twelve (112) clients within a thirty(30) mile radius. To speak to you about
<br /> the sober living we are proposing, those of you that may not know what a sober living is, it is an
<br /> out-patient supervised and structured living environment that works in combination with evidence-
<br /> based treatment, one (1) on one (1) counseling, one (1) on one (1) case management, provided
<br /> transportation, physical fitness, nutrition and workforce preparation. Choices Recovery currently
<br /> tracks all of its patients for one(1)year through our After-Care Department. Not only do we reach
<br /> out to the individual clients, but we reach out to every immediate family member they release on
<br /> their information to gather information from them. Through the data we have collected through
<br /> our After-Care Department, we see that our clients are two and a half(2.5) times more likely to
<br /> stay sober if they attend a sober living facility, like the one (1) that is being proposed today, as
<br /> opposed to returning home.
<br /> Mr. Adolf went on, I think even more importantly, as we have tracked through our clients,
<br /> unfortunately, to the disease of addiction, which many times left untreated is deadly, we had five
<br /> (5)patients die after receiving treatment. All of those patients returned immediately home and did
<br /> not go to a sober living facility. Unfortunately,there are little to no resources in our area. Although
<br /> we do have similar facilities like Upper Room, Dismas House, Jane's House, and Oxford House,
<br /> all of these are full or near-full with many of them having a waiting list, and none of these offers
<br /> evidence-based treatment in combination. The large majority of these resources are strictly
<br /> housing. I would like to read our sober living mission statement.Our sober living program supports
<br /> recovery and stability from drug addiction, by providing and reinforcing a positive healing
<br /> environment. Detoxification and substance abuse treatment are only the beginning in the recovery
<br /> process. It is our goal to assist beyond treatment and provide residents the foundation in skills they
<br /> need to achieve complete physical, emotional and spiritual health that can last a lifetime.
<br /> Mr. Adolf continued, I would now like you to look through your packet. We have attached a
<br /> combination of pictures for you,hoping to give you a little bit of visual evidence to go along with
<br /> the facility. If you look at picture one (1) provided to you in the packet, that would be an example
<br /> of what a bedroom would look like at this new facility. Picture two (2) is also an example of what
<br /> a bedroom would look like at this new facility. Getting to picture three (3) gets to the security
<br /> systems that are currently in place at Choices Recovery. Although our patients are all there
<br /> voluntarily, we do everything we can in precautions by providing a high-tech security system and
<br /> fully-trained security staff, twenty-hour (24) hours a day, three hundred and sixty-five (365) days
<br /> a year. If you look at picture three (3), that is the keypad and intercom that is the entrance to our
<br /> facility. Picture four (4) is a picture of our current gate at Choices Recovery. Picture five (5) is a
<br /> photo of an exterior camera that we have installed at Choices Recovery. Picture six (6) is a photo
<br /> of an interior camera. Picture seven(7)is, ultimately, our security system screen. This exact same
<br /> camera system and video system would be installed at the new facility. We are looking at an
<br /> estimated fifteen(15) cameras to provide full coverage for the interior and exterior of the building.
<br /> All of this video will be backed up to a hard drive that would be stored for at least one(1) year. If
<br /> there are any incidents that occur, we can clip those and save those onto another drive for review.
<br /> Mr. Adolf went on, In addition to the security system, we plan on having two (2) to three(3) staff
<br /> members for each shift to provide twenty-four (24) hour coverage, seven (7) days a week, three
