MWBE Good Faith Effort Forms must be completed, signed, and handed in with the bid <br />submission. These forms are shown in the bid proposal, Contractor's Bid for Public <br />Works, sheets 9, 10, and 11. <br />Payments for work completed during this project will be made every thirty (30) calendar <br />days. Consistent with provisions of IC 36-1-12-14, the Board of Public Works shall retain <br />a percentage of payments throughout the duration of the project. <br />Materials and properties purchased under contract with the Owner that becomes a <br />permanent part of the structure or facilities constructed are not subject to the Indiana <br />Gross Retail Tax (Sales Tax). The exemption number will be furnished to the Contractor <br />upon award. <br />V. Comments by Department of Public Works <br />Questions due by 5:00 p.m. Local Time on 12/11/18. <br />All work in the City of South Bend Right of way shall use current City standards. <br />All Contractors AND subcontractors must be pre -qualified for being a responsible bidder <br />as of July 2018. The Contractor is responsible for completing the checklist and attaching <br />all requested documents in the bid package. The Checklist can be found under General <br />Conditions starting on page 6. <br />VI. Contractor Questions <br />Why is there a 50-calendar day limitation for projects prior to April 1s'? Please see <br />Addendum No. 1 for clarification. <br />
