1. All Contractors and subcontractors doing business with the City of South Bend shall <br />present a Certificate of Insurance showing coverage in the following minimum amount: <br />a. General Liability: Premises -Completed Operations or Products, Bodily Injury and <br />Property Damage Combined Single Limit - $5,000,000. <br />b. There shall be no exclusion for explosion, collapse or underground hazard. <br />c. Workmen's Compensation: Statutory State of Indiana Employer's Liability - $100,000. <br />d. Auto Liability: Bodily Injury and Property Damage Combined Single limit - <br />$1,000,000 <br />e. City of South Bend shall be named as additional insured on the Certificate of Insurance. <br />f. Contractor and subcontractors shall provide any and all insurance policies, documents, <br />endorsements, or certificates related to this Contract immediately upon the request of <br />the City of South Bend. <br />g. If the Contractor is a joint venture or a limited liability company, the insurance policy <br />must name the joint venture or limited liability company as a named insured. <br />h. The policies reflecting this insurance coverage shall endorsed to provide the City with <br />at least thirty (30) days written notice of reduction, cancellation, or intent not to renew <br />such coverage. <br />i. If insurance is cancelled, reduced, non -renewed or otherwise not in effect to the <br />minimum required coverage, the Contractor shall notify the City and shall cease work <br />on the Project. <br />j. When any insurance policy is renewed or replaced, the policy retroactive date shall <br />coincide with or precede start of work on the contract. <br />k. When any asbestos abatement work is performed in connection with this contract, the <br />Contractor or its subcontractor performing such work shall provide Asbestos Liability <br />Insurance with limits of not less than $1,000,000 per occurrence insuring bodily injury, <br />property damage and environmental cleanup. <br />I. The Contractor is responsible for all loss or damage to personal property, including <br />materials, equipment, tools and supplies, owned, rented or used by Contractor. <br />VIII. AWARD OF CONTRACT <br />A. Prevailing Specifications: 2018, INDOT Standard Specifications Section 103 <br />B. Additions: <br />1. All Bids will remain subject to acceptance for sixty (60) calendar days after the day of the <br />Bid opening, but the City of South Bend may, in its sole discretion, release any Bid and <br />return the Bid security prior to that date. <br />2. Successful bidder from award notice will have fourteen (14) calendar days to submit a fully <br />executed contract, Certificated of Insurance, and other require documents from either the <br />awarded contactor and/or the subcontractors. Failure to comply within the award period <br />may be cause for the Board of Public Works to rescind the award. <br />Version 8/16/2018 Special Provisions - 3 <br />
