<br />AWARD QUOTATION — ANGELA BRIDGE LIGHTING — PROJECT NO. 116-111 (COIT)
<br />Ms. Alicia Czarnecki, Engineering, advised the Board that on August 14, 2018, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Ms.
<br />Czarnecki recommended that the Board award the contract to the lowest responsive and
<br />responsible bidder, Martell Electric, LLC, 4601 Cleveland Road, South Bend, IN 46628, in the
<br />amount of $49,022. Therefore, Ms. Dorau made a motion that the recommendation be accepted
<br />and the quotation be awarded as outlined above. Mr. Mueller seconded the motion, which
<br />carried.
<br />APPROVE CHANGE ORDER NO. 3 — IRONWOOD DRIVE PAVEMENT
<br />REHABILITATION AND INTERSECTION — PROJECT NO. 117-071 A (LOCAL ROAD
<br />AND BRIDGE GRANT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 3 on behalf
<br />of C&E Excavating, Inc., 53767 County Road 9, Elkhart, IN 46514, indicating the contract
<br />completion date be extended by thirty-two (32) days, for a new contract completion date of
<br />September 25, 2018. Upon a motion made by Ms. Dorau, seconded by Mr. Mueller and carried,
<br />the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — TUCKER DRIVE — PROJECT NO. 116-118A (RWDA
<br />TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, IN 46614, indicating the contract
<br />amount be increased by $25,230.91 for a new contract sum, including this Change Order, in the
<br />amount of $309,203.68. Upon a motion made by Ms. Dorau, seconded by Mr. Mueller and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — RUM VILLAGE DOG PARK CONSTRUCTION
<br />(2015 PARK BOND)
<br />Mr. Gilot advised that Mr. Patrick Sherman, Venues, Parks, and Arts, has submitted Change
<br />Order No. 1 on behalf of R. Yoder Construction, Inc., 27453 CR 150, Nappanee, IN 46550,
<br />indicating the contract amount be increased by $9,775 for a new contract sum, including this
<br />Change Order, in the amount of $130,180. Upon a motion made by Ms. Dorau, seconded by Mr.
<br />Mueller and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — CITY CEMETERY PATH RESURFACING (COIT)
<br />Mr. Gilot advised that Mr. Patrick Sherman, Venues, Parks and Arts, has submitted Change
<br />Order No. 1 on behalf of Acorn Landscaping LLC, 3680 W. Sample Street, -South Bend, IN
<br />46619, indicating the contract amount be increased by $3,000 for a new contract sum, including
<br />this Change Order, in the amount of $49,830. Upon a motion made by Ms. Dorau, seconded by
<br />Mr. Mueller and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 4 — DOWNTOWN CROSS STREET IMPROVEMENTS —
<br />PROJECT NO. 117-041 A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 4 on behalf
<br />of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, IN 46614, indicating the contract
<br />amount be increased by $25,250.62 and an additional eight (8) days, for a new contract sum,
<br />including this Change Order, in the amount of $1,697,189.22 and a new project completion date
<br />of November 3, 2018. Upon a motion made by Ms. Dorau, seconded by Mr. Mueller and
<br />carried, the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — MORRIS PERFORMING ARTS
<br />CENTER MARQUEE DISPLAY INSTALLATION — PROJECT NO. 117-070A (MORRIS
<br />CAPITAL IMPROVEMENT FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Burkhart Sign Systems, LLC, 1247 Mishawaka Avenue, South Bend, IN
<br />46615, for the above referenced project, indicating a final cost of $34,600. Upon a motion made
<br />by Ms. Dorau, seconded by Mr. Mueller and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — WEST RACE EMERGENCY REPAIRS
<br />— PROJECT NO. 115-115 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of HRP Construction Co., 5777 Cleveland Road, South Bend, IN 46628, for
