IV. WAGE RATES <br />A. Prevailing Specifications: 2010, INDOT Standard Specifications Section 103 <br />B. Additions: <br />1. The attached wage rates are applicable to this project. The successful Bidder is required <br />to submit Form 347, Weekly Payroll form, to the EEO Compliance Officer within two (2) <br />weeks of the applicable payroll date. <br />2. The Contractor shall post at the job site any and all information required to comply with <br />EEO guidelines. <br />3. The Contractor, and any sub - contractors, shall submit a schedule of wages to be paid on <br />the project prior to beginning work on the site. <br />V. TAX EXEMPT <br />A. Prevailing Specifications: None <br />B. Additions: <br />Materials and properties purchased under contract with the Owner that becomes a <br />permanent part of the structure or facilities constructed are not subject to the Indiana <br />Gross Retail Tax (Sales Tax). The exemption number will be furnished to the Contractor <br />upon award. <br />VI. PROSECUTION AND PROGRESS <br />A. Prevailing Specifications: 2010, INDOT Standard Specifications Section 108 <br />B. Additions: <br />1. The completion time for this project is thirty (30) calendar days from Notice to Proceed. <br />2. The Contractor will send the Notice of Demolition to IDEM and EPA upon the Owner's <br />notice of award. Notice to Proceed will be effective upon the end of the ten working day <br />notification period for the notice of demolition. The IDEM notification form shall be <br />provided as included in these specifications. <br />3. Contractor shall provide a schedule of demolition prior to beginning any work on the site. <br />VII. RETAINAGE AND FINAL PAYMENT <br />A. Prevailing Specifications: 2010, INDOT Standard Specifications Section 109 <br />B. Additions: <br />1. Payments may be requested once every calendar month. <br />2. There will be a five percent (5 %) retainage. <br />3. Before final payment is released, the Contractor must satisfy the following: <br />a. All parts and labor meet requirements stated in the specifications. <br />b. Provide copies of test reports or cut sheets on all materials supplied. <br />C. One (1) copy of the Completion Affidavit and one (1) copy of a Waiver of Lien are <br />submitted to the City of South Bend. Department of Public Works. <br />VIII. CONCRETE APPROACH, SIDEWALK, AND CURB REMOVAL <br />A. Prevailing Specifications: 2010. INDOT, Standard Specifications Section 202 <br />B. Additions: <br />All curb, sidewalk, and approaches surrounding the site is to REMAIN. Any damage by <br />the contractor to the existing curb, sidewalk, or approaches shall be replaced at the <br />owner's discretion at no additional cost to the owner or increase in contract amount. <br />
