Street The project includes the furnishing of all equipment, material, and labor for the demolition of two buildings located at 622 and 718 East Sample Street within the City of South Bend. Work to be performed shall include the removal of multiple structures, and all other appurtent work on the site as specified in these documents. Compact fills and subbase materials to 100% standard proctor maximum dry density. Testing will be done by nuclear gauge. Testing will be completed by an independent testing agency. All costs associated with material testing will be considered incidental to the project. Testing locations shall be chosen by the owner or owner's representative. The number of tests will be at the discretion of the owner based upon compaction methods and fill material. PREVAILING SPECIFICATIONS AND DESIGN & CONSTRUCTION STANDARDS The City of South Bend's PREVAILING SPECIFICATIONS, most recent version, and DESIGN & CONSTRUCTION STANDARDS, most recent version, are to be used on this project. Each bidder is specifically instructed to become completely familiar with the PREVAILING SPECIFICATIONS and the DESIGN & CONSTRUCTION STANDARDS prior to submitting a bid. These SPECIAL PROVISIONS will list only "Additions" or "Deletions" to the PREVAILING SPECIFICATIONS and are to be used only in conjunction with the PREVAILING SPECIFICATIONS. INSURANCE A. Prevailing Specifications: 2010, INDOT Standard Specifications Section 103 B. Additions: All Contractors and subcontractors doing business with the City of South Bend shall present a Certificate of Insurance showing coverage in the following minimum amount: General Liability: Premises - Completed Operations or Products, Bodily Injury and Property Damage Combined Single Limit - $5,000,000 or $750,000 per person. There shall be no exclusion for explosion, collapse or underground hazard. Workmen's Compensation: Statutory State of Indiana Employer's Liability - $100,000. Auto Liability: Bodily Injury and Property Damage Combined Single limit - $1.000,000 City of South Bend shall be named as additional insured on the Certificate. the date of final completion as set forth in the final completion affidavit.
