required more frequently by the discharge permit, a periodic <br /> compliance report on forms provided or prescribed by the Director <br /> indicatin� the nature and concentration of prohibited or re u� lated <br /> substances in the effluent. Included in the report shall be a record <br /> of all measured or estimated avera�e and maximum daily flows <br /> durin� the reportin�period, computed by verifiable techniques as <br /> required b ty he applicable discharge permit. In cases where the <br /> Pretreatment Standard requires compliance with a Best <br /> Management Practice (BMP) or pollution prevention alternative, <br /> the User must submit documentation required by the Director or <br /> the Pretreatment Standard necessary to determine the compliance <br /> status of the User. <br /> b. Users shall be required to have a Certified Industrial <br /> Wastewater Operator of the State of Indiana si�n all periodic <br /> compliance reports indicatin�Lwhether pretreatment Standards and <br /> Requirements are bein� met on a consistent basis, and if not <br /> whether additional operation and maintenance and/or additional <br /> pretreatment is required to meet the pretreatment standards and <br /> requirements, unless otherwise indicated in the User's permit. The <br /> Director shall specify the certification class required in the User's <br /> permit• <br /> c. All periodic compliance reports must be si�ned and <br /> certified in accordance with Subsection (p)(5)(a) of this Section 17- <br /> 10 and si n� ed bv an Authorized Representative as defined in <br /> Section 17-5.1, Division 1 of this Article. <br /> d. All wastewater samples must be representative of the User's <br /> discharge. Wastewater monitoring and flow measurement facilities <br /> shall be properlv operated, kept clean, and maintained in good <br /> workin� order at all times. The failure of a User to keep its <br /> monitorin fg_ acilit�good workin� order shall not be �rounds for <br /> the User to claim that sample results are unrepresentative of its <br /> dischar�e. <br /> e. If a User subject to the reportin� requirement in this section <br /> monitors any regulated pollutant at the appropriate samplin� <br /> location more frequently than required by the Director, usin.�he <br /> procedures prescribed in Subsection (�L1), "Analvtical <br /> Requirements" and in Subsection (p)(2) "Sample Collection" of <br /> this Section 17-10, the results of this monitoring shall be included <br /> in the report. <br /> 49 <br />
