EXHIBIT B <br />South Bend Memorial Hospital Bartlett Street Parking Structure <br />Facade Renovation <br />Carl Walker Scope of Services <br />A. Design Update Phase - We understand that South Bend Memorial Hospital and the <br />City of South Bend have agreed upon the facade design as shown in Exhibit A. Prior to <br />issuing this design for bidding, the Design Update Phase will provide an opportunity to <br />confirm agreement on the proposed facade concept, confirm existing <br />conditions /configuration, update the code review and address required modifications, <br />develop construction controls necessary to maintain parking operations, develop front <br />end specification requirements (Divisions 0 and 1), update estimated construction costs, <br />and finalize the 2010 project schedule. <br />1. The initial work effort will involve compiling and distributing the most recent design <br />information, including design assumptions and the 2006 design documents. <br />2. Meet with the City and the Hospital to establish project requirements, establish the <br />schedule, and to set milestone dates. <br />3. Attend, chair and document two (2) design coordination meetings with the City and <br />Hospital. <br />4. Present the proposed architectural concept to the City of South Bend. We understand <br />that site plan approval will not be required for this project. <br />5. Review building code /zoning changes that may affect construction and cost. We <br />propose to retain RTM (Code Consultant) to assist with this review. Our current scope <br />does not include the preparation of variance applications, if required. <br />6. Meet with City building officials to review our understanding of specific code <br />requirements and their impact on the project. <br />7. Construction Planning /Coordination - We will meet with the City and Hospital to <br />develop specific project requirements for construction controls and phasing. As a <br />minimum, the following items will be discussed: <br />a. Required contractor construction sequencing to minimize disruption to <br />parking and adjacent buildings. <br />b. Review Hospital requirements or concerns that will affect construction such as <br />noise restrictions dust and fume controls, required construction signage, <br />barricades, construction safety, etc. <br />8. ' Prepare front end specifications (Division 0 and Division 1) based on City requirements. <br />9. Prepare final architectural and structural plans, details, and specifications required for <br />the architectural improvements. <br />10. Submit required documents for review to the appropriate government authorities. <br />0 1 <br />11. Confirm the construction cost estimate and the anticipated construction schedule. <br />
