REGULAR MEETING <br />JUNE 12, 2018 178 <br />1 <br />f� <br />1 <br />KABELIN COMMERCIAL SUPPLY <br />512 Andrew Ave. <br />LaPorte, IN 46350 <br />Quotation was submitted by Kristin Moss <br />INTERBORO PACKAGING CORPORATION <br />114 Bracken Rd. <br />Montgomery, New York 12549 <br />Quotation was submitted by Tony Friedman <br />Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the above quotations <br />were referred to Central Services for review and recommendation. <br />OPENING OF QUOTATIONS — MISHAWAKA AVENUE — ROTO ROOTER JETTER <br />REMOVAL —PROJECT NO. 118-069 (SEWER CAPITAL) <br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed quotations for <br />the above referenced project. Ms. Martin, Clerk of the Board, stated no quotations were received. <br />Mr. Gilot stated the Sewer Department could go to the open market for quotes to get the work <br />done. <br />OPENING OF QUOTATIONS — EAST RACE SIDEWALK REPAIR — PROJECT NO 117- <br />130 (CURB & WALK SIDEWALK/SMALL DRAINAGE) <br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed quotations for <br />the above referenced project. Ms. Martin, Clerk of the Board, stated no quotations were received. <br />Mr. Gilot stated the Engineering Department could go to the open market for quotes to get the <br />work done. <br />TABLE BID AWARD — RAVINA PARK — BOWMAN CREEK STABLIZATION — PROJECT <br />NO. 117-073 (MVH) <br />Ms. Alicia Czarnecki, Engineering, advised the Board that on May 8, 2018 bids were received <br />and opened for the above referenced project. After reviewing those bids, Ms. Czarnecki <br />recommended that the Board award the contract to the lowest responsive and responsible bidder <br />HRP Construction, Inc., 5777 Cleveland Road, South Bend, IN 46624 in the amount of <br />$57,191.47 for Division Nos. 2 and 3. Mr. Jered Baker, HRP Construction, was present and <br />informed the Board that when they bid on this project, their bid was based on all three (3) <br />divisions, they did not know it was divisible. He noted Division No. 1 is the large part of the <br />project that includes the riverbank stabilization, and the other two (2) parts are only sidewalk <br />installation. He stated HRP is not a sidewalk contractor and they would therefore need to sub -out <br />all of the work. Mr. Baker stated they have made numerous attempts to contact the City <br />Engineers to discuss this but have not been able to get a response, so he is hoping to open the <br />discussion with the Board. Mr. Gilot stated their overhead should have been distributed across <br />the whole project. He suggested the City Engineering Office and the Legal Department should <br />meet with Mr. Baker and discuss. Therefore, Ms. Maradik made a motion that the bid award be <br />tabled. Ms. Maradik seconded the motion, which carried. <br />AWARD BID — EIGHT (8) MORE OR LESS CARDIAC MONITORS/DEFIBRILLATORS <br />(EMS CAPITAL) <br />Mr. Andy Myer, Fire Department, advised the Board that on May 22, 2018, bids were received <br />and opened for the above referenced equipment. After reviewing those bids, Mr. Myer <br />recommended that the Board award the contract to the lowest responsive and responsible bidder <br />Physio-Control, Inc., 11811 Willows Road NE, Redmond, WA 98052 in the amount of <br />$26,018.49 each with options and eight (8) trade in's for a total of $208,147.92. Therefore, Ms. <br />Maradik made a motion that the recommendation be accepted and the bid be awarded as outlined <br />above. Ms. Dorau seconded the motion, which carried. <br />AWARD BID — TWO (2) MORE OR LESS 2018 OR NEWER REFURBISHED MEDIC <br />UNITS — SPEC Q (FIRE DEPARTMENT CAPITAL) <br />Mr. Jeff Hudak, Central Services, advised the Board that on May 22, 2018, bids were received <br />and opened for the above referenced equipment. After reviewing those bids, Mr. Hudak <br />recommended that the Board award the contract to the lowest responsive and responsible bidder <br />Crossroads Ambulance Sales & Service, LLC, 52886 State Road 13, Middlebury, IN 46540 in <br />the amount of $352,826 including the international MV chassis.
