This is EXHIBIT A, consisting of 6 pages, referred to in <br />and part of the Agreement between Owner and Engineer <br />for Professional Services dated M, ay 22, 2018. <br />Engineer's Services <br />Article 1 of the Agreement is supplemented to include the following agreement of the parties. <br />Engineer shall provide Basic and Additional Services as set forth below. <br />PART I — BASK SERVICES <br />This project will include an assessment of the sewer infrastructure in some of the areas that experienced <br />flooding along the St. Joseph River during the February 2018 historic flood event. These areas include the <br />Northshore Triangle, Keller Park, and some neighborhoods along Riverside Drive. This assessment will <br />include: <br />• Gather data from the residents regarding flooding in their homes and on their properties. <br />• Review current, available GIS and as -built data to identify potential issues with the existing <br />system. <br />• Perform a field investigation of select locations to verify GIS and/or as -built data. <br />• Provide a summary and assessment of the impact of the 2018 historic flooding event. Also <br />provide data and a summary of the 2016 and 2018 storm events including the storm <br />characteristics and comparison to design storm events. <br />• Determine potential improvements to the system. <br />• Prepare a report and attend a public meeting to present the findings. <br />Al .01 Data Gathering and Review of Existing Information <br />A. LFA will prepare a questionnaire to be distributed online to residents within the study areas. The <br />responses to this questionnaire will be summarized and used to assist with the determination <br />of issues and potential remediation within the study areas. <br />B. LFA will review the City's GIS data of the study area and compare the information with as -built <br />drawings. Discrepancies between the two sets of data and areas requiring further investigation <br />will be identified. <br />C. LFA will review video of the sewer systems within the study areas and utilize the information to <br />determine potential problems and to verify the GIS and as -built data. The sewer videos will be <br />provided to LFA by the City of South Bend. <br />D. LFA will prepare a Field Investigation and Survey Plan to document the items inthe field <br />requiring additional information and further verification. <br />E. LFA will meet with the City to discuss the proposed field work identified in the Field <br />Investigation and Survey Plan. <br />A1.02 Field Investigation and Survey <br />A. LFA will investigate the areas identified in the Field Investigation and Survey Plan. This may <br />require confined space entry.
