<br />AWARD BID — WASTEWATER TREATMENT PLANT BELT FILTER PRESS NO. 1
<br />REHABILITATION — PROJECT NO. 118-019 (UTILITIES, WASTEWATER, OTHER
<br />EQUIPMENT)
<br />Mr. Jacob Klosinski, Utilities, advised the Board that on April 24, 2018, bids were received and
<br />opened for the above referenced project. After reviewing those bids, Mr. Klosinski
<br />recommended that the Board award the contract to the lowest responsive and responsible bidder
<br />Alfa Laval, Inc., 10470 Deer Trail Drive, Houston, TX 77038 in the amount not to exceed
<br />$203,131. Therefore, Ms. Dorau made a motion that the recommendation be accepted and the bid
<br />be awarded as outlined above. Mr. Mueller seconded the motion, which carried.
<br />AWARD BID — ONE (1) OR MORE, 2018 OR NEWER, TRASH TOTE CLEANING
<br />TRAILER SYSTEM — SPEC M (SOLID WASTE CAPITAL LEASE)
<br />Mr. Jeff Hudak, Central Services, advised the Board that on April 24, 2018, bids were received
<br />and opened for the above referenced equipment. After reviewing those bids, Mr. Hudak
<br />recommended that the Board award the contract to the lowest responsive and responsible bidder
<br />Hydro -Chem Systems, Inc., 6605 Broadmoor Ave SE, Caledonia, MI 49316 in the amount not to
<br />exceed $69,890. Therefore, Ms. Dorau made a motion that the recommendation be accepted and
<br />the bid be awarded as outlined above. Mr. Mueller seconded the motion, which carried.
<br />AWARD QUOTATION — WEST RACE EMERGENCY REPAIR— PROJECT NO. 115-115
<br />(HOTEL/MOTEL TAX)
<br />Ms. Sue Ellen Doudrick, Engineering, advised the Board that on May 3, 2018, one (1) quotation
<br />was received and opened for the above referenced project. After reviewing the quotation, Ms.
<br />Doudrick recommended that the Board award the contract to the sole responsive and responsible
<br />bidder, HRP Construction, Inc., 5777 Cleveland Road, PO Box 266, South Bend, IN 46624, in
<br />the amount of $84,100 for the Base Bid plus Alternate B. Therefore, Ms. Dorau made a motion
<br />that the recommendation be accepted and the quotation be awarded as outlined above. Mr.
<br />Mueller seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO. 1 — CENTURY CENTER VESTIBULE HEATER
<br />REPLACEMENT — PROJECT NO. 117-044 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Edward J. White, Inc., 1011 S. Michigan Street, South Bend, IN 46601, indicating the contract
<br />amount be increased by $1,945.74 for a modified contract sum, including this Change Order, in
<br />the amount of $34,150.74. Upon a motion made by Ms. Dorau, seconded by Mr. Mueller and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 3 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— EDISON AND IRONWOOD ROAD CORRIDOR REHABILITATION — PROJECT NO.
<br />116-070 LOCAL ROAD AND BRIDGE MATCHING GRANT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 3 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc. indicating the contract amount be decreased by
<br />$91,376.34 for anew contract sum, including this Change Order, of $1,330,517.10. Additionally
<br />submitted was the Project Completion Affidavit indicating this new final cost of $1,330,517.10.
<br />Upon a motion made by Ms. Dorau, seconded by Mr. Mueller and carried, Change Order No. 3
<br />(Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — 2017 ENGINEERING REMODEL —
<br />PROJECT NO. 117-116 (BUILDING AND STREET MAINTENANCE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Dr., South Bend, IN
<br />46628, for the above referenced project, indicating a final cost of $13,392. Upon a motion made
<br />by Ms. Dorau, seconded by Mr. Mueller and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER COOLING
<br />TOWER REPLACEMENT —PROJECT NO. 117-035 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Edward J. White, Inc., 1011 S. Michigan St., South Bend, IN 46601, for
<br />the above referenced project, indicating a final cost of $226,631. Upon a motion made by Ms.
<br />Dorau, seconded by Mr. Mueller and carried, the Project Completion Affidavit was approved.
