The Practice <br />has the option of splitting the payment of the Initial Investment into two. The first payment of $4,800.00 will be <br />due within 30 days of the initial invoice. The remaining balance and second payment of $4,800.00 will be <br />invoiced after the completion of the implementation of the EHR upon the Practice having its first meaningful <br />use of the EHR, or three (3) months following the Effective Date, whichever is sooner. <br />Invoicing for the Annual Subscription Fee will be invoiced in full upon the Practice's first full month of productive <br />use of the EHR. <br />MHIN Form #3013 Revised: 0810812014 Page 14 of 19 <br />
