121 <br />REGULAR MEETING MAY 22, 1978 <br />USE OF FIRE TRUCK IN MEMORIAL DAY PARADE APPROVED <br />Chief Oscar Van Wiele submitted to the Board a request to use a <br />reserve fire truck in the West Side Veteran's Memorial Day <br />Parade to be held on Monday, May 29 and his recommendation that <br />the request be approved. Upon motion made by Mr. McMahon, seconded <br />by Mr. Mullen and carried, the request was approved. <br />PUBLIC HEARINGS SCHEDULED ON REVENUE SHARING REQUESTS <br />Mr. McMahon requested that the Board approve advertising for public <br />hearings on revenue sharing requests of $70,000 for a vacuum type <br />street sweeper and $5,000 for roof repairs at the Municipal Services <br />Facility. Upon motion made by Mr. McMahon, seconded by Mr. Mullen <br />and carried, the Board approved the request and directed the Clerk <br />to advertise with the public hearings to be held on Monday, June 5. <br />CHANGE ORDER APPROVED - CURB AND SIDEWALK CONTRACT I, 1977 <br />Change Order No. 1 in the 1977 Curb Improvements, ContractoI, was <br />submitted to the Board. The Change Order increases the contract by <br />$36,656.15 and covers additional sidewalk, pavement and curb removal. <br />Areas involved are on Clayton Drive, Coral Drive, the intersection of <br />Klinger and Addison and St. Joseph Street. Upon motion made by <br />Mr. McMahon, seconded by Mr. Mullen and carried, the Change Order <br />was approved. <br />CHANGE ORDER AND PROJECT COMPLETION AFFIDIVAT APPROVED - PROSPECT <br />STREET SEWER PROJECT <br />A Final Change Order in the Prospect Street Sanitary Sewer Contract <br />was submitted to the Board. The Change Order decreases the contract <br />by $932.50 and is to adjust the contract to as -built plans. Upon <br />motion made by Mr. McMahon, seconded by Mr. Mullen and carried, the <br />Change Order was approved. The Project Completion Affidavit for the <br />project was submitted. The Contractor was Marion Hamilton & Sons, <br />Inc. and the total amount of the contract was $16,290.90. A Maintenance <br />Bond was submitted, with the maintenance agreement to extend until <br />May 15, 1981. Upon motion made by Mr. McMahon, seconded by Mr. Mullen <br />and carried, the Project Completion Affidavit was approved and the <br />Maintenance Bond was filed. <br />CHANGE ORDER APPROVED - MODEL CITIES SEWER CONTRACT <br />Change Order No. 5 in the Model Cities Sewer Contract was submitted <br />to the Board. The Change Order extends the calendar days for <br />completion of the contract by 134 days but does not change the amount <br />of the contract. Mr. McMahon explained that it is a requirement of <br />HUD that any change of days in a contract must be officially approved <br />by Change Order. The change order extends the date of the contract <br />to March 23, 1978. Upon motion made by Mr. McMahon, seconded by Mr. <br />Brunner and carried, the Change Order was approved. <br />CONTRACT APPROVED - CREST MANOR SEWERS <br />The contract with Taylor Construction Company for the Crest Manor <br />Section 2 sanitary sewers was submitted to the Board. The contract <br />was let in November but because of the weather construction was not <br />started. The contract and performance bond are now ready for execution, <br />and construction will start this week. Upon motion made by Mr. <br />McMahon, seconded by Mr. Mullen and carried, the contract was approved. <br />
