Zawlocki, of Midland Corporation non - collusion affidavit was in 911 Cleveland Street order, and a 5% bid bond was Mishawaka, Indiana submitted. Metal Fabrications - Bid Package #108 $95,500.00 Mr. McMahon stated that certain alternates outlined in the bid proposal called for a reduction in the bid amount, but also omitted certain items which were specified. He indicated that the total amount of the omissions amounted to a reduction of $29,875.00. St. Joseph Valley Structural Bid was signed by Dean R. Kelly, Steel Corporation non - collusion affidavit was in 1203 Kent Street order, and a 5% bid bond was Elkhart, Indiana submitted. Metal Fabrications - Bid Package #108 $102,700.00 Mr. David Jahn of Sollitt Construction Company explained that the budget amount for the metal fabrications project was $20,500.00. Upon a motion made by Mr. McMahon, seconded by Mr. Kernan and carried, the bids were rejected. OPENING OF BIDS - AMBULANCE AND RELATED EQUIPMENT This was the date set for receiving bids for an ambulance and related equipment. The Clerk tendered proofs of publication of notice in the South Bend Tribune and the Tri- County News which were found to be sufficient. Box 733 non- collusion affidavit was in Carmel, Indiana order, and a 10% bid bond was submitted. Type 1 Ambulance, aluminum construction, mounted on 1980 Chevrolet chassis $41, 969.00 each Trade -in allowance $1,000.00 Alternate Bid #1 Type 1 Ambulance, aluminum construction, mounted on 1980 Chevrolet chassis $45,530.00 Trade -in allowance $1,000.00 Plus optional equipment. Butler Coach Company, Inc. 8401 West Washington Street Indianapolis, Indiana 46231 Bid was signed by Jack R. Trusty, non - collusion affidavit was in order, and a 10% bid bond was attached. Type 1 Ambulance, modular type vehicle with 1980 Chevrolet chassis (modular body built by Horton Company) Trade -in allowance $1,500.00 *Type 1 Ambulance, modular type vehicle 1980 Chevrolet, aluminum construction (modular body built by Wheeled Coach) Trade -in allowance $1,500.00 Plus optional equipment. $41,558.00 $40,532.00 *It was noted that the alternate bid takes complete exception to the specifications.
