Jefferson Blvd. Suite 1200 -S <br />South Bend, IN 46601 <br />Code Compliance Work for Clubhouse Relocation <br />2602 Western Ave., South Bend <br />Dear Mr. Relos: <br />The following is our proposal to provide professional design services for architectural work as <br />required to convert the former Kolacz Market building at the corner of Western Ave. and Meade <br />into a clubhouse for the Hell's Angels motorcycle club. <br />SCOPE OF WORK <br />1. ADG, Inc. will measure the existing building (3 levels) and draw existing floor plans. The <br />drawings will be of sufficient detail to allow for code review by local and state building officials. <br />2. ADG will design architectural / structural improvements needed to bring the building into code <br />compliance for the change of occupancy in the building. Improvements will include: redesign of <br />the entrances and addition of accessible restrooms. <br />3. ADG will meet with local code officials to review the proposed work. ADG will assist the City <br />in submitting the project to the Department of Homeland Security in Indianapolis in order to gain <br />state approval for the work. <br />4. Based on the approved design, ADG will prepare bid specifications so that the project may be <br />competitively bid by the City of South Bend. If required, ADG will incorporate the city's front end <br />documents, including wage rate information, into the project manual. <br />5. During construction, ADG will perform the minimum construction inspection and <br />administration work as required by the state to assure that the work is being done in compliance <br />with the approved plans and specifications. <br />
