Contractor Application for Payment <br />• Review and agree to pay quantities with Contractor prior to submitting <br />applications for payment <br />• Check Contractor's applications for payment, sign and transmit payment <br />applications to the Bureau of Construction Manager. <br />Change Orders <br />• CBBEL staff will coordinate and process change orders in a format acceptable <br />to the City of South Bend. <br />Project Close -Out <br />• Prepare a preliminary Punch List <br />• Conduct a Punch List Project Walk- through with the City <br />• Prepare and transmit a Final Project Punch List to the Contractor <br />• Verify completion of Punch List items <br />• Assemble Final Project Files and Documents <br />• Review Final Application for Payment and prepare Final Change Order <br />ESTIMATED FEE <br />We have estimated the total fee for these services to be $15,300. This total fee is <br />estimated to be divided as follows: <br />Sub - Contractor Services <br />Environmental Inspection Services $ 500 <br />(Phifer Environmental Services, LLC) <br />CBBEL Services <br />Construction Observation Demolition $ 9,700 <br />Total <br />Construction Observation Curb & Sidewalk 5,100 <br />$ 15,300 <br />If the value of work accomplished exceeds $15,300, CBBEL will assess the remaining <br />work and will notify you, in writing, of the revised compensation and schedule before <br />continuing with the services. No additional work will be performed until a written contact <br />amendment has been approved by both parties. <br />We will bill you monthly, on a time and material basis, for assigned tasks in accordance <br />with our attached standard Charges for Professional Services. In addition, our contract <br />will be established in accordance with the attached General Terms and Conditions. <br />These General Terms and Conditions are expressly incorporated into and are an integral <br />part of this contract for professional services. <br />Christopher B.
