Burke Engineering, Ltd. ( CBBEL) is pleased to submit this proposal to <br />provide part-time construction observation services for the demolition of the property at <br />236 Sample Street and for the installation of curb and sidewalk on Sample and Carroll in <br />South Bend, Indiana. CBBEL proposes to furnish the services described below for each <br />phase separately in order to accommodate the each phase's individual schedule. <br />SCOPE OF SERVICES <br />Services to be provided by CBBEL for this work have been identified as follows: <br />Pre - Construction Conference <br />• Organize and set agenda for Conference <br />• Notify representatives of the City, Contractor, and all utilities <br />• Develop and distribute meeting minutes and sign -in sheet <br />Field Reports, Records, and Daily Work <br />For the purpose of this task, CBBEL staff have assumed a thirty (30) calendar day <br />construction period for the demolition and a thirty -five (35) calendar day construction <br />period for the curb and sidewalk, based on the Contract Documents. CBBEL staff will <br />provide part time construction observation for the noted activities: <br />• Inform Contractor of observed deficiency in Contractor's work <br />• Complete a Daily Report for each day's work in the format desired by the City <br />and provide copies to the Bureau of Construction Manager. <br />• Collect and forward Certified Payrolls /Payroll Vouchers to the Bureau of <br />Construction Manager <br />3 Christopher B.
