City of South Bend <br />April 15, 2010 <br />Page2of3 <br />PHASE I ENVIRONMENTAL SITE ASSESMENT REPORT <br />The objective of the Phase I Environmental Site Assessment is to identify environmental concerns <br />based on recognized environmental conditions in connection with the subject property located at <br />507 W. Western Avenue (parcels #018-3012-0424, #018-3012-0425 and #018-3012-041201). <br />The proposed services will be performed in accordance with the American Society for Testing <br />and Materials (ASTM) document ASTM E 1527-05. <br />The proposed Phase I Environmental Site Assessment will consist of the following components: <br />• Records Review <br />• Site Reconnaissance <br />• Interviews <br />• Reporting <br />Total Phase I Environmental Assessment Report $1,700.00 <br />DEMOLITION SPECIFICATION PREPARATION <br />Prepare and provide the City of South Bend a set of demolition specifications ready for bidding <br />in order to demolish the property at 507 W. Western Avenue as described by the information <br />provided by the City. The specifications will include site demolition and restoration instructions, <br />instructions on the removal of any environmental items identified in the Phase I Environmental <br />Site Assessment, any asbestos identified in the asbestos survey, and any additional <br />environmental issues identified during the site inspection. <br />Total Demolition Specification Preparation Service Fees $7,600.00 <br />DEMOLITION INSPECTION AND CONSTRUCTION ENGINEERING SERVICES <br />Wightman Petrie will provide one part time on-site Project Inspector whose duties will include <br />demolition inspection (4 hours per day), writing daily reports, tracking all pay item quantities for <br />verification purposes, making field determinations on workmanship of various items within the <br />project, and acting as a liaison and first point of contact for the General Contractor's <br />representative, affected property and business owners, City officials, the Design Engineer, and <br />all other interested parties to the project. <br />In addition, Wightman Petrie's Design Engineer will provide overview of the project, reviewing <br />and approving of inspector's weekly reports to the City, visiting the construction site during key <br />activities, assisting inspector in quality control review, issuing work directives, helping find <br />solutions or answers to questions or problems that may occur during construction, and issuing <br />the certificate of substantial completion and assisting with the closure documentation. <br />The following estimates are provided based on a 45-caldendar day sequence: <br />
