<br />APPROVE PROJECT COMPLETION AFFIDAVIT — WASTEWATER TREATMENT
<br />PLANT COMPRESSOR BUILDING ROOF REPLACEMENT - PROJECT NO. 116-052
<br />(UTILITIES, WASTEWATER, MAINTENANCE SERVICES BUILDING)
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Environmental Services, has submitted the Project
<br />Completion Affidavit on behalf of Dudeck Roofing & Sheet Metal, 1634 Franklin St., South
<br />Bend, IN 46613, for the above referenced project, indicating a final cost of $251,600. Upon a
<br />motion made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — ONE (1) OR
<br />MORE 2018 OR NEWER 3/4 TON FOUR WHEEL DRIVE PICK UP TRUCKS — SPEC D
<br />(MVH)
<br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced vehicles. Therefore, upon a motion
<br />made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above request was approved.

<br />advertise for the receipt of bids for the above referenced vehicles. Therefore, upon a motion
<br />made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above request was approved.

<br />In a memorandum to the Board, Mr. Roger Nawrot, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. Fritzberg and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — SALE OF
<br />ABANDONED VEHICLES
<br />Ms. Kathy Eli, Department of Code Enforcement, requested permission to advertise for the sale
<br />of approximately sixteen (16) abandoned vehicles, which are being stored at Super Auto Salvage
<br />Corporation, 201 West Sherwood, South Bend, Indiana. It was noted that all vehicles have been
<br />stored more than fifteen (15) days, identification checks have been run for auto theft, and the
<br />owners and lienholders notified. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />Fritzberg, and carried, the above request was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — TUCKER DRIVE — PROJECT NO 116-118A (RWDA TIF)
<br />In a memorandum to the Board, Mr. Roger Nawrot, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. Fritzberg and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS 2018 STREET
<br />AND SEWER DEPARTMENT MATERIALS — PROJECT NO 118-031 (STREETS CURB

<br />In a memorandum to the Board, Mr. Scott Kreeger, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced materials. Therefore, upon a motion
<br />made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above request was approved.
