City of SOUdi Send <br />EAP Contract Page 1 <br />Prepared on November 14, 2017, Amended January 17, 201 S <br />1. Overview <br />New Avenues will arrange for the delivery of Employee Assistance Program services to <br />all employees and their dependents as indicated in the Definition of Eligibility addendum <br />during the term of this Agreement with City of South Bend hereinafter to be referred as <br />"the Company." New Avenues will provide employee assistance services as hereinafter <br />described. New Avenues will arrange for such services to be provided by duly trained and <br />qualified professionals in accordance with generally recognized treatment standards. <br />11. Location and Service Access Information <br />Our corporate office address is P.O. Box 360, South Bend, Indiana 46624. Our corporate <br />office location is 300 South St. Louis Boulevard, Suite 200, South Bend, Indiana 46617. <br />New Avenues manages a network of EAP Providers across the state of Indiana and <br />nationwide to provide service to your employees. <br />A. Any employee or immediate family member may be seen in any of our network <br />offices near their home or workplace. <br />B. Hours of Operation for corporate office and member services include: 8:30 a.m. to <br />5:00 p.m., Monday through Thursday: 8:30 a.m. to 4:00 p.m., Friday (Eastern <br />Standard Time or Eastern Daylight Savings Time during the appropriate times of <br />the year), Our on -call clinical staff is available for immediate or emergency <br />situations 24 hours per day including weekends and holidays. <br />C. A toll -free telephone number will be provided to all employees for easy EAP <br />access. <br />III. Access Procedure and Orientation Service <br />A. Members can access their EAP services by calling New Avenues and speaking to <br />an Intake Specialist who will obtain necessary account information and information <br />of the member's counseling needs. A Care Manager (Master's level mental health <br />counselor) is available for members to consult when dealing with higher risk and <br />emergency needs. The member will then be informed how counseling services will <br />proceed. New Avenues will make a reasonable attempt to select an EAP Provider <br />that can provide continuity of care with the member's health plan if needed. <br />B. Members of our professional staff will meet with all employees setup at the <br />Company's request, in groups, at your sites for approximately 30 minutes to orient <br />theirs to the program, including how to self -refer, stress confidentiality, and <br />normalize problems so that persons will feel comfortable making appointments. <br />We suggest group sizes of approximately 35-40 employees at a time. <br />C. We will provide you with a draft introductory letter announcing the EAP, to be <br />mailed by you to the home of each employee introducing the EAP program. <br />New Avenues <br />
