<br />This section of the proposal should establish the ability of vendor(s) to satisfactorily perform the required work by
<br />reasons of: experience in performing work of a similar nature for similar clients (size and account base) within the past
<br />five years; demonstrated competence in, the services to, be provided; educational qualifications; strength and stability
<br />of the firm; staffing capability; work load; record of meeting schedules on similar projects; and supportive client
<br />references. Vendor shall complete Proposal Form A as part of this section. If the proposed solution is a packaged
<br />partnership, then the significant sub contractors shall also provide information for this section. Vendor must provide
<br />additional iinformation in this section that will:
<br />(1) provide a brief profile of the firm(s), including the types of services offered; the year founded; form of the
<br />organization (corporation, partnership, sole proprietorship); number, size, and location of offices; inumber, of
<br />employees;
<br />(2) provide a general description of the firm s,financial condition; identify any conditions (e.g., bankruptcy, pending
<br />litigation, planned office closures, impending merger) that may impede vendoes ability to complete the project of
<br />meet the vendors obligation during the first five years of maintenance and support;
<br />(4) describe the firm's experience in performing work of a, similar nature to that solicited in this RFP within the past 5
<br />years for clients of similar size and accounts, and highlight the, participation in such work by the key personnel i
<br />proposed for assignment to this project; and
<br />(6) identify sub -consultants by company name, address, contact person, telephone number, and project function;
<br />describe vendors experience working with each sub -consultant.
