P_ <br />282 <br />5. Attending physicians will not submit endorsement (statement) to the <br />city controller until such time as treatment or physical examination <br />is completed. Separate forms will be executed for separate injuries <br />or examinations. <br />6. In the event it is necessary for one of the physicians to make a <br />house call, or emergency room call, -the authorizing person will <br />make the appointment by phone and immediately issue a form of <br />authorization to the physician by mail. <br />7. Under no circumstances will one of the three physicians treat or <br />examine any city employee without proper authorization. <br />8. Those persons permitted to authorize medical treatment will be <br />required to execute a first report of injury form as prescribed <br />by the Workments Compensation Laws of Indiana. This form shall <br />be forwarded to the city personnel director within two days <br />after the injury. <br />9. The attending city physician will be requested to complete a <br />surgeonts report as prescribed by'Workments Compensation Laws of <br />Indiana. This report will be forwarded to the city personnel <br />director. <br />10. All employees shall be required to report all injuries and illness <br />to their supervisors. <br />11. Prisoners in custody of City Police shall be treated in the same <br />manner as any city employee whenever practicable. <br />12:. Physicianis basic rates shall be: <br />4.00 per Office Call <br />`5.00 per House Call <br />,5.00 Fire & Police Physical and pre -employment <br />examination. <br />(The above does not include medication or minor surgical procedure.) <br />The Board named the following to have authority to sign the authorization form to <br />CD <br />be executed for any case ' requirring medical treatment for injury or illness arising <br />from performance of duty, and for medical examination: <br />J. A. Bickel <br />H. M. Tomlinson <br />Clifford Lake <br />R. J. Gillen <br />Y: J Voelkel <br />George Hinkle <br />Richard Stickley <br />Chas. Kite <br />R. J. Duck <br />A.'O. Brown <br />Wm.. J. Kiel <br />E. H. r_iller <br />Excavation Bonds, each in the sum of :1,000.00, were examined and approved for <br />the following contractors: <br />Anthony Kujawski & Son <br />Earl C. De Van <br />John M. Rensberger <br />Contractorts Bond, in the sum of $1,000.00, for Edward Mathews, Mishawaka, Indiana <br />was examined and approved. <br />The Bond of Charles Aulm, General Contractor, in the sum of <br />repairs to Ironwood Bridge as per plans and specifications by Wm. <br />.examined and approved by the Board. <br />$18,000.00, to cover <br />S.
