Indiana Professional Engineer No. 10300102 CONTRACTOR: Selge Construction PROJECT: Western Avenue Corridor Improvements Project No. 11S-097 SUBJECT: Change Order 2 ISSUE DATE: October 10, 2017 This Change Order to the Contract document shall supplement, amend, and shall become a part of, and be incorporated as an Addendum to the Contract. The following is a summary and description of the revisions to the Contract by this Change Order: Phase III: South Side Improvements — july 31, T September 15, 2017 Phase IV: North Side Improvements — September 15 2017 OGt ti, r 40L, 2 October 26, 2017 Phase V: Fixtures, Trees, Fencing, and Landscaping — OGtGbGF 13, 2 October 31, 2017 The completion dates were extended because: 1) The contractor is adding another layer to the Pave Drain under bed (Add 4 days: 2 South Side, 2 North Side). 2) The pavement section at the intersection of Olive & Western on the south side was adjusted (0.5 days) 3) The contractor extended a larger water service than what was called for in the plans (add 1 day) 4) The contractor encountered fiber optic ducts in the way of the storm sewer and needed to raise the fiber optic ducts and existing manhole. (add 1 day) 5) The contractor encountered fiber optic under the light foundations and required extra time to install reinforcement around the conduit. (add 2 days) 6) The contractor performed additional work to address voids under the pavement (add 2.5 days) 7) Additional walk on South Side of Kaley Street (add 1 day) The overall final completion date was changed to November 10, 2017, Total Change Order Request: $85,534.18 SHEET 1 OF 4
