Change Order No. 01 <br />2017 Demolitions, Phase I <br />Project No. 117-065 <br />The following sections of the Contract Documents are revised as a result of the above changes: <br />BIDDING AND CONTRACT REQUIREMENTS: <br />Bid/Proposal Form: <br />o DIVISION A: 414 N OLIVE STREET <br />■ REMOVAL IN ITS ENTIRETY <br />o DIVISION C: 3602 W WESTERN AVENUE <br />REMOVE <br />4 1 ENVIRONMENTAL ALLOWANCE 1 1 LS $ 10,000.00 $ 10,000.00 <br />= AD D: <br />6 BUILDING REPAIR 1 LS $ 17,454.00 $ 17,454.00 <br />SPECIFICATIONS: <br />The following special provisions have been amended or added to the Contract Documents and are issued <br />as part of this Change Order: <br />XIV. PROSECUTION AND PROGRESS <br />A. Prevailing Specifications: 2016, INDOT Standard Specifications Sec. 108 <br />B. Additions: <br />1. The project will have a completion date of sixty (60 100) calendar clays for all work. The <br />contract time will start when the Notice to Proceed is delivered and signed. <br />2. The Contractor shall have twenty-four (24) calendar days to complete demolition work on <br />each site once work has begun on that site. The Contractor shall have twenty-four (14) <br />calendar days to complete building repair work on each site once building repair work has <br />begun on that site. <br />XLIII. BUILDING REPAIR <br />A. Prevailing Specifications: None <br />B. Additions: <br />1. Work to be performed shall include furnishing all labor, services, materials, Insurance and <br />equipment to install vinyl siding and repair features associated with the east face wall of <br />3610 Western Avenue. Such work shall include: <br />a. Insulating the existing exposed wall cavity with batt insulation. <br />b. Furnishing and installing new plywood sheathing to the exterior of the wall. <br />SHEET 2 OF 3 <br />
