The following quotations were opened and read: <br />DUDECK ROOFING AND SHEET METAL, INC. <br />1634 Franklin Street <br />South Bend, IN 46613 <br />Quotation was submitted by Gregory Dudeck <br />Contractor's Non -Collusion Affidavit, Non -Debarment Affidavit, Certification Regarding <br />Investment with Iran, Employment Eligibility Verification, Non -Discrimination <br />Commitment for Contractors and Certification of Use of United States Steel Products or <br />Foundry Products was submitted. <br />QUOTATION: $21,500.00 <br />Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the above quotations <br />were referred to Venues, Parks, and Arts (VPA) for review and recommendation. After <br />reviewing those quotations, Mr. Patrick Sherman, VPA, recommended that the Board award the <br />contract to the sole responsive and responsible bidder, Dudeck Roofing and Sheet Metal, Inc., in <br />the amount of $21,500.
